New Member Enrollment Information

Effective immediately, all new members of the North Attleborough Contributory Retirement System must contact the System's office to complete the necessary paperwork for membership in the System.

Office hours are Monday through Friday, 8:30 am to 4 pm. Walk-ins are welcomed, however, we recommend you contact the System directly at 508-699-0119 to set up an appointment.

The following documents are required in order to complete the New Member Enrollment form as well as the beneficiary forms:

  • Birth Certificates:
    • Yours
    • Spouse (if applicable)
    • Children
  • Marriage certificate (if applicable)
  • Divorce Decrees and/or Domestic Relation Orders (if divorced)
  • Veteran Status (if applicable):
    • DD-214
    • NGB 23B (if applicable)

Staff will be able to assist you in completing the forms as well as answer any questions that you may have about the System.

The office is conveniently located in the Tower Square Building at the corner of Route 1 and Elm Street.
