Attach a copy of your contract signed and dated by parties. Attach a copy of both sides of the contract if the terms are on the back. Contracts that are more than 3 years old will not be accepted. The contract must list the location, the size of each container, the number of recycling containers, and when and how often each container is serviced. The contract must list each location serviced by this contract.
If your business is in North Attleboro, the hauler must be licensed by the North Attleboro Board of Health. Also if your business is located in North Attleboro, please supply a copy of your current North Attleboro Board of Health Permit.
If you do not own the business then you must provide, in addition to the contract, a letter from the business owner who has the trash/recycling contract giving you permission to use the containers.
